Inputs checklist

Clause 8.9.2 lists the inputs a review must record, items a to p. Each appears as a line with a “covered” tick and a note. They include changes in internal and external issues, fulfilment of objectives, suitability of policies and procedures, the status of actions from previous reviews, outcomes of recent internal audits, corrective actions, assessments by external bodies, changes in the volume and type of work, customer and personnel feedback, complaints, effectiveness of improvements, adequacy of resources, results of risk identification, outcomes of the assurance of the validity of results, and other relevant factors such as monitoring and training.

Most of the evidence for these inputs is already in AccreditBook: nonconformities, audit findings, PT outcomes and complaints raised as nonconformities. Write your conclusion on each in its note.

Outputs checklist

Clause 8.9.3 asks for outputs on at least: the effectiveness of the management system and its processes, improvement of laboratory activities related to the requirements of the standard, provision of required resources, and any need for change. Each is a line with a note.

Approving the minutes

Approve minutes moves the review to Minutes approved. The Activity tab records who approved and when, and every edit before and after.

Due dates

Next review due appears in the dashboard due list. Management reviews have a 30-day grace period: a review a few weeks late shows as Overdue, and only after 30 days past the date as Lapsed.

First week

Enter your most recent review with its date, attendees and minutes, and set the next review date. If the last review’s minutes do not cover every input, the unticked lines show you what to cover next time.

What the assessor looks for

Records showing every required input was considered, outputs with decisions and actions, and actions followed up at the next review. The review and its minutes go into the assessor pack.
