Nonconformities

How to raise a nonconformity, what each state means, the allowed transitions, and why Close stays disabled until an action is effective.

Screen: Nonconformities Clauses 7.10, 8.7 Updated

The nonconformities screen is your NCR log. It covers nonconforming work (clause 7.10) and the corrective actions that follow (clause 8.7). See Clause 7.10 and Clause 8.7.

The list

Each NCR shows its number, source, description, state chip and age in days. Numbers are assigned by AccreditBook in sequence, for example NCR-2026-014.

Fields

Field What to enter
Source Internal audit, External assessment, PT result, Complaint, Equipment, Process or Other.
Raised on, raised by Date and person.
Description What happened, with objective evidence.
Immediate action What you did straight away to contain it.
Work affected Which results, reports or customers may be affected.
Customer notified Yes or no.
Risk level Low, Medium or High.

When an NCR is raised from an audit finding or a PT result, the source is linked and shown at the top.

States and transitions

From Can move to
Open Investigation, Cancelled
Investigation Action planned, Cancelled
Action planned Action in progress, Cancelled
Action in progress Verification
Verification Closed, or back to Action in progress

Any other move is refused. This keeps the record honest: an NCR cannot jump from Open to Closed.

Why Close is disabled

An NCR can only be closed when at least one corrective action on it is completed and its effectiveness check is recorded as Effective. Until then the Close button is disabled and the reason is shown next to it. If the check finds the action was not effective, move the NCR back to Action in progress and add another action. See Corrective actions.

What the assessor looks for

That nonconforming work was identified, its significance evaluated, affected work recalled or customers told where needed, root cause found, action taken and its effectiveness verified. Every state change is in the Activity tab with a name and time, and the whole log goes into the assessor pack.

First week

Enter open NCRs from your current log with their real raised dates, so ages are correct. Closed NCRs from the current cycle are worth entering too, because the assessor will ask about them. Members can create and edit NCRs but cannot delete records created by someone else. A deleted NCR is kept in the history and in a full data export.
